Send out ballet slipper shaped invitations. For a unique birthday invitation print the party invitation on pink paper and place inside a ballet slipper, wrap in tulle, and place into a mailable box. You can make tutus for each of your guests using some ribbon, tulle, and fabric glue.
You can set up a simple ballet studio for the party. If you are handy you could visit your local home improvement store and purchase some simple mirrors and a stair rail.
